Couldn't find anything either but it looks like a typical photo-interrupter module.
You forward bias the LED with a small DC current, likely 10-20mA, and bias the photo transistor with 5V, through a 50-100kΩ load resistor.
Then when the gap between is blocked, the transistor is off, and when not blocked, the transistor is on.

Nothing else on it other than the date code from 1984 35 week.
