Need help identifying and finding a replacement one of these

It's kind of hard to know for sure without knowing the manufacturer. As Diver said the 150 is most likely 150µF. The C could be a tolerance or voltage rating of 16V. The rest is anyone's guess, date code, lot number, etc. Is it bad or did it just come out of something and you want to replace it with a new one?
 
According to the label and size it is a 150µF/6VDC (surge 8V at 85deg/C) electrolytic cap. The brand name is most probably "Jamicon".

Here is a datasheet of Vishay-Sprague chip electrolytic caps. Their enclosure differs from the Jamicon. If length is suitable you might choose a Vishay-Sprague type.

How did you know? I don't understand how to read capacitors sometimes, especially ceramic disk capacitors.
