5 pins when you include the power pins. Most op amps are like this so you shouldn't have any trouble finding one.

It wouldn't really be much of an opamp if it only had a single input - it would have to be an opamp pre-wired to a particular configuration in which case I think the overall device would be called by a different name - buffer, amplifier, etc.

Four pins?
I can't see an op-amp on the site you linked with only four pins.

I don't see how it's posibble to have an op-amp with any less than four pins.
