See the motherboard pinout chart in the Wikipedia article.
Use a 3.5mm stereo plug in each socket and test continuity from the plug terminals to the wires / pins.
The mic plug tip goes to header pin 1
Mic plug ring to pin 3
Headphone plug tip to pin 9,
Headphone plug ring to pin 5.
Sleeve / ground for both to pin 2 (preferably separate wires as far at the motherboard header).
Then with no plugs in, see if there is continuity between any spare wires/pins and header pins 5 or 9.
If there is, it's "AC97" wiring and the pin 5 return goes to pin 6, with pin 9 return to pin 10.
If no continuity to 5 or 9, try combinations of unused wires for any with continuity between them. If there are, try a plug in each socket to see if that causes them to disconnect.
If it does, it's HD Audio mode and the floating switch appears to go between ground and 6 for the mic switch, or ground and 10 for the headphone switch..
