1. how to create ECG signal in the Multsim?

Use the Piecewise Linear source.

Some people PLAY a heartbeat at about 16Hz but it is actually a DC pulse (thump thump), not an AC frequency (boom boom).

Use the multisim wav input option [ like the LTspice option], down load from the web Heartbeat Sounds in wav file format and run the wav sounds into your simulation.

This method works OK in LTspice.
